Overview:
Proper documentation is crucial in all aspects of your practice.
It will help you to get properly reimbursed by insurance companies and is an integral aspect of providing quality care to all of your clients.
Topics to be covered include: taking a thorough history and physical, necessity of the HIPPA, Oswestry and Neck Disability forms, S.O.A.P. notes, client sign-in sheet, reevaluations and more.
Each registrant will receive a Client Intake Form, History and Physical Form, HIPPA Form, Oswestry Questionnaire and a Neck Disability Form, all of which can be used in their clinics on Monday morning!

Dr. Fronczak is a 1999 graduate of the New York Chiropractic College. He also earned a B.S. in Biology from St. John Fisher College. Dr. Fronczak is the A.C.T. Program Coordinator at NYIM.
