NATIONALS – You can download an application by going to www.ncbtmb.com. You will need to submit a "Request for Transcript" form to NYIM and we will send your transcript directly to the Nationals (we cannot send the official transcripts directly to you). Fill out the application completely and enclose the following: your photograph, a photocopy of a current government issued identification, method of payment, and a notarized copy of your diploma. On question #6 use School Code #031846 and on #12 select Contemporary Western. Please use the checklist provided in the Nationals handbook. Send your packet certified to cover yourself in case it is lost in the mail. Processing usually takes 3-4 weeks.
